- Latest J/EA Form
- Verification through the ‘HRMIS’ System for government servants
- Original S&P Agreement/ booking sheet/ down payment receipt
- Business Registration and other related forms for self-employed (whichever related)
- Any additional documents (if required) – Latest EPF statement
